NIHR BioResource Questionnaire - neurological disease or operation list

The NIHR BioResource questionnaire includes a question asking for details of any neurological disorder or operation. This is submitted as a free text entry field. Initially we don't expect to be able to code this data, so a free text entry will be used. If patterns emerge we can review this.
